All commercial email should be required to say ADV The reply-to address MUST be the address used to opt-out Email must be checked at least every 15 minutes to ensure the email box doesn't get full If email is still sent after you opt-out, then each individual should be able to sue for a specified amount. If a company simply changes the email address and continues sending spam, they should be fined a large amount (that REALLY means something, so that even a large company such as Microsoft would be hurt by the fine, instead of simply seeing the fine as a "cost of doing business".
