Subject: A simple question

I have a simple procedural question.

I will be making a filing hopefully on Tuesday. My question concerns the Certification. If our client will be swearing under penalty of perjury, does he sign in both boxes ? Or does he just sign the second box ?

Wishing everyone a nice long weekend.
