Ordinary course document are not 4(c) for the party that prepares them. But could be for the other party if a copy is given to them.

You need to determine who actually has beneficial ownership of the shares. If that is the bank or the trust, they would be listed in 6(b). If beneficial ownership resides with the investors/employees...

You need only produce 4(c) documents related to the exercise of the option in 2006.
